By submitting a form on the ERUS, site, you give consent to the Operator (ERUS) to process your personal data in order to handle the booking request and contact you to arrange an appointment time.

• phone number;
• email address;
• information from the Comment field.

Actions with data

Collection, recording, storage, updating, use, and deletion to the extent necessary for the stated purposes.

Term and withdrawal

Consent remains valid until the processing purposes are achieved or until it is withdrawn. You can withdraw consent at ermedical@yandex.com.
